The mission of CareCo Medical Home Health is to provide comprehensive, compassionate, and customized in-home medical care designed to help individuals remain in their homes, safe and strong.

It is also where we want to be when we feel sick, injured, or vulnerable. When we feel like that we can sometimes find ourselves out of the safe space we call home.

 

Today, you can receive an almost limitless range of health care services that allow you or a loved one to stay right at home. At CareCo Medical Home Health, we can help make that achievable by working closely with you. Our clinical staff can provide direct skilled services to you or a loved one while helping coordinate care alongside family, doctors, and other healthcare professionals. With CareCo Medical as your home health provider, an experienced clinician will develop a customized plan of care made specifically for you based on your diagnosis and health concerns.

Skilled Nursing is a generalized title that refers to medical care being delivered by licensed nurses. At CareCo Medical Home Health skilled nursing services are provided by registered nurses (RN) and/or licensed practical nurses (LPN). LPN’S work closely under the supervision of an RN. Skilled nursing provides many medical services based on what is deemed medically necessary by your primary care doctor.

Rehabilitative therapies are an essential part of recovering from an acute illness or injury. At CareCo Medical Home Health we have physical therapists (PT) and occupational therapists (OT), based on what your rehabilitative needs are provided by your doctor we will assign the perfect therapist to you. At CareCo our goals are patient centered meaning our therapy plans are designed to integrate your diagnosis and medical needs along with your personal therapeutic goals. With CareCo Medical’s therapists by your side you can conquer the goals that are important to you.

Home Health Aides at CareCo Medical Home Health are Certified Nursing Assistants (CNA) who work under the supervision of a registered nurse (RN). They can provide you with different services that can help your at home life be a little bit easier. Some tasks Home Health Aides can provide are bathing, assisting with transfers, etc. Home Health Aides do not run errands or house keeping but CareCo Medical can give you a referral to some personal care aide (PCA) agencies that would fit your individual needs.
